The ledger connects the request, work, evidence, judgment, and acceptance without replacing the systems around it.

People decide which findings become policies, playbooks, and documentation for future work.

This retail example shows the proposed model inside one organization.

Work begins. A store opens a support ticket in Jira. An Oracle replenishment agent checks the order and stock. An Informatica master-data agent checks the vendor terms.
People apply judgment. Frank, the business owner, defines the exception path. Patty, the category manager, authorizes it. Power Automate executes the authorized integration batch.
The ledger preserves the account. It links the ticket, inventory facts, and vendor terms; records the action and outcome; keeps Frank's rationale; and records Patty's approval.
The organization updates its standard. Governance documents the rationale, approval, and outcome; defines the scope, limits, and authority; then publishes the policy, playbook, and agent guidance.

Motus starts local. Later stages remain proposals until real use justifies them.
